Public Health Solutions’ Harlem Health Advocacy Partners (HHAP) program is a place-based Community Health Worker intervention aimed at improving health and neighborhood conditions of New York City Housing Authority (NYCHA) residents in East and Central Harlem. The program’s purpose is to bridge the gap between healthcare and social services and Harlem NYCHA residents, particularly those facing barriers such as limited access to healthcare, cultural and linguistic differences, and socioeconomic challenges. CHWs serve as the vehicle between the residents, communities, healthcare providers and service requests; helping to promote health education, prevention, and access to care. 

Program Assistance

Health Coaching

One-on-one coaching to NYCHA residents to help manage chronic conditions, including asthma, diabetes, and high blood pressure. Community Health Workers (CHWs) support residents to set health goals, provide education and guidance to help meet those goals, and address their questions and concerns about health and wellness. They also facilitate communication with healthcare providers and connect residents with social services and resources. Health coaching is available to any NYCHA resident living in zip codes 10029, 10035, or King Towers, and managing asthma, tobacco cessation, diabetes, and/or high blood pressure.

Educational Wellness Workshops

CHWs are assigned to NYCHA developments to conduct regular wellness activities (walking groups, blood pressure monitoring) and workshops with the residents (e.g. fitness classes, falls prevention, stress management). 

Community Outreach, Engagement, and Advocacy

CHWs are tasked with identifying and establishing connections with NYCHA residents, local organizations, and community leaders to build trust and rapport. They also organize and participate in community health fairs, awareness campaigns, targeting canvassing and door knocking, tabling, and other events to reach residents in the community. We engage residents in at least 3 advocacy campaigns per year, planned and implemented within a community organizing framework.   

Navigating the Healthcare System

We partner with Single Stop’s Health Advocates who can help you find affordable or low-cost health insurance, read medical bills and review health plan options.

HHAP Works!

HHAP has provided one-on-one health coaching to over 450 Harlem NYCHA residents since November 2022, made more than 400 referrals to supports in the community, and held over 700 wellness events with over 6,000 participants attending.

HHAP participants have also demonstrated improvements in blood pressure control, blood sugar control, mental health, and self-reported general health among program participants. After their participation in HHAP, most residents retain resources they continue to use.
