Expansion of Community Resource Network to Improve the Social Determinants of Health during the COVID-19 Pandemic

With support from resources raised by the NYC COVID-19 Rapid Response Coalition, PHS will support CBOs that can help expand the range of services available and/or geographic coverage of the Unite NYC network. Selected CBO partners will engage in a formalized system of partners that streamlines services and improves the quality and accountability of referrals provided to clients. PHS will serve as the backbone organization for this system, providing capacity building and technical assistance aimed at supporting sustained participation in a Community Resource Network. Performance data collected from the platform will be used to build NYC's human service capacity and to advocate for funds for additional resources where gaps are identified. A valuable aspect of participation in this community resource network is that your organization’s work will be reflected when opportunities arise for a regional community resource network or value-based opportunity.

Organizational Eligibility Requirements

• Be legally incorporated by the New York State Department of State as a not-for-profit corporation;
• Have federal tax-exempt status under Section 501 (c)(3) of the Internal Revenue Code; and
• Provide services in New York City
