Overview
Family Connect (FC) is a community resource network for families who are currently pregnant or have a child under the age of 5 years old. Family Connect navigators screen families for various needs and connect them to community resources that help them and their family thrive. Some of the resources the Family Connect navigators refer families to are home visiting programs, doula services, food support, prenatal or postpartum support groups, childcare, housing support, mental health services, and more. Through the Family Connect EMPATHS initiative, navigators conduct an enhanced mental health screening and refer to a variety of mental health services, including the FC Social Worker.
