Overview

WholeYouNYC is a free community resource network of services that improve the conditions that affect people’s health. Our ultimate goal is to create “one-stop” for community resources that:

  • Makes it easier for New Yorkers to find, apply for, and receive needed services.
  • Improves participants’ health.
  • Reduces reliance on and costs to the health system.

Request for Application

Thank you for your interest in joining the Social Care Network. The current Request for Applications (RFA) is closed.

We encourage interested organizations to check back periodically, as we may reopen the application process or invite new partners if specific network gaps or community needs emerge.

About the WholeYouNYC Social Care Network

A connected system of care holds the key to addressing disparities and improving health outcomes in our communities. We connect healthcare providers and community-based organizations to ensure that New Yorkers can access the services and resources they need to lead healthier lives.

The WholeYouNYC Social Care Network includes 300+ community-based service providers that deliver more than 460 unique programs across Brooklyn, Queens, and Manhattan. By joining our network, organizations gain access to a wide range of resources to help deliver more effective and coordinated care. We support each partner by making resources more accessible and creating a platform for community-wide improvement.

Our Network partners—Health-Related Social Needs (HRSN) Service Providers—play a critical role in delivering services that improve health outcomes of Medicaid members. These partners may include, but are not limited to:

  • Community-based organizations addressing essential social needs such as housing, food security, and transportation
  • Health care providers offering wraparound services that address their patients’ social drivers of health
  • If your organization is committed to improving health outcomes of Medicaid members and supporting underserved communities, joining the Network offers a meaningful opportunity to expand your impact.

By becoming a part of the WholeYouNYC network, your organization will:

  • Receive financial support through Medicaid reimbursement under the 1115 Waiver.
  • Gain access to a collaborative network of healthcare and social service providers, helping you better coordinate care.
  • Support the expansion of your services to more individuals and families in need across Brooklyn, Queens, and Manhattan.
  • Help shape the future of integrated care by addressing the social needs that often hinder positive health outcomes.
  • Benefit from training, technical assistance, and resources to help your organization thrive in this new care model.

Application Process

The application process is open-ended and ongoing. We continuously accept applications from eligible service providers who are interested in joining the network.
