The WholeYouNYC Social Care Network
Our Social Care Network is a cornerstone of the New York State Department of Health 1115 Medicaid Waiver, designed to transform how health and social care work together across communities.
Launched in January 2025, the Network has reached more than 500,000 Medicaid members across Queens, Brooklyn, and Manhattan—and continues to grow toward serving over 2 million people. By bringing together more than 300 health care providers and community-based organizations, we are building a coordinated ecosystem that addresses both medical needs and the social drivers of health.
Through the Social Care Network, members can more easily access critical services such as care management, food and nutrition support, housing assistance, and transportation—resources that are fundamental to health and well-being but have historically been fragmented or difficult to navigate.
At the same time, we invest in the strength and sustainability of our partners. The Network provides funding, training, technical assistance, and capacity-building support to community-based organizations and providers, enabling them to deliver high-quality, culturally responsive services at scale.
Together, we are building a more connected system—one that meets people where they are, addresses social drivers of health, and creates the conditions for individuals and communities to thrive.

Within 48 hours, a navigator will get in touch to let you know we received your submission and find out more about you and your situation.

The navigator will connect you to personalized resources, through a variety of network partners, that fit your needs and preferences.
Am I Eligible?
Every New Yorker who is actively enrolled in Medicaid can be screened for health-related social needs (HSRN). HRSN services include food and nutrition, housing and rental assistance, and transportation.
To be eligible for enhanced services – which provide more support – Mediaid members must also be enrolled in a Medicaid Managed Care plan* and meet specific requirements.**
To be eligible for enhanced HRSN services you must be:
- Enrolled in a Medicaid Managed Care plan,* and
- Meet the clinical and/or social risk threshold for the service, and
- Have an unmet need based on the program screener, and
- Belong to an enhanced care group
You may belong to an Enhanced Care Group if you are:
- Experiencing frequent ER visits or hospital stays in the past 12 months
- Pregnant or gave birth within the last 12 months
- Enrolled in the Medicaid Health Home program
- Recently released from jail or prison within the last 90 days and have a qualifying health condition
- Living with a substance use disorder
- Living with a serious mental illness
- Living with an intellectual or developmental disability and need support
- A child under 18 who are at risk for chronic physical, developmental, behavioral, or emotional conditions like asthma, obesity, child abuse or neglect, low birth weight, or autism, amongst other conditions.
After you complete the self-screener, a Social Care Navigator will reach out to you to review your answers and discuss what you are eligible for.

Medicaid Member Advisory Council
The Medicaid Member Advisory Council (MMAC) is a new initiative aimed at improving Medicaid services and member experience. The council provides an opportunity for Medicaid members to share their experiences, challenges, and feedback to guide decisions and improve services.
