Support
We offer many different free services to help NYC children, parents and pregnant moms.
Overview
Our Family Connect program helps families access a wide range of resources during pregnancy and after their children are born.
PHS’s Community Resources offer group activities for parents and families, along with free diaper and crib distribution.
Our Home Visiting Programs provide families with ongoing support in the places where they live.
We also partner with health care systems throughout the city to support maternal and child health.
Family Connect
(Assistance for all pregnant and parenting families)
Family Connect is a community resource network for families who are pregnant or have a child under the age of 5 years old and connect them to community resources and mental health support that help them and their family thrive, including:
- Home visiting programs
- Doula services
- Food support
- Prenatal or postpartum support groups
- Childcare
- Housing support
- Mental health services including short-term therapy

Community Resources
PHS offers many programs that help New York families and kids thrive. From support groups and peer education to virtual support, resource hubs, and more, these community-based programs connect families with vital tools, information, services and support.

Home Visiting Programs
Our voluntary home visiting programs offer free, in-home health and parenting education, as well as group support, from Nurse Home Visitors, Family Support Specialists, Community Health Workers, and Health Coaches.
Participation in our programs helps pregnant and parenting New Yorkers:
- Have a safer and healthier pregnancy and birth.
- Learn about birth justice and respectful care.
- Learn how to care for themselves and their babies during the postpartum period.
- Learn how to make their homes safer for children and reduce injuries.
- Connect to support if feeling down or depressed.
- Plan future pregnancies and reduce the chance of becoming pregnant if they don’t want to be.
- Learn child-friendly games and developmental activities to increase school readiness.

Community-Health Care Collaborations
for Maternal Child Health
PHS partners with health care systems to support healthier outcomes for our city’s moms and kids. Current partnership goals include:
Overcoming Barriers to Maternal Healthy Weight: The CHIME Study
In partnership with Stanford University, PHS is the lead community partner on a five-year research grant from the Patient Centered Outcomes Research Institute to advance maternal health equity. The Comparing Health Interventions for Maternal Equity (CHIME) study compares the effectiveness of three different approaches to overcoming barriers to maternal healthy weight and preventing chronic disease by improving cardiovascular and metabolic health and wellness during and after pregnancy.

Supporting Pregnant and Pediatric Patients
In partnership with NewYork-Presbyterian/Queens Hospital, PHS’s Pediatric Community Health Worker and OB Community Health Worker programs help patients and caregivers achieve their best outcomes through support, education and coordination of clinical and community care. The program is for NewYork-Presbyterian patients only.
