Partner Spotlight: Coordinated Behavioral Care

For more than 15 years, Coordinated Behavioral Care has been helping community organizations do what they do best, making it easier to deliver care that truly changes lives. Founded to build one of New York’s first behavioral health networks, Coordinated Behavioral Care (CBC) brings together over 70 community-based behavioral health, housing, and human service organizations. CBC was created to tackle challenges that no single provider should have to solve alone. Care management for people with complex needs, smoother transitions from hospital to community, and sustained, compassionate support to help people experiencing homelessness move into safe, stable places they can call home. 

Over the years, CBC has become a trusted backbone for community care. Through programs such as Health Home Care Management, Pathway Home, Safe Options Support, and Housing Location and Placement Services, CBC and its network agencies, supports tens of thousands of New Yorkers each year. Improving stability, reducing avoidable hospital use, and strengthening connections across health and social care systems. Just as importantly, CBC invests in the organizations and staff who make that work possible, offering shared infrastructure, data, training, and hands‑on support that lighten the administrative burden and strengthen frontline impact.

CBC’s partnership with Public Health Solutions’ Social Care Network reflects this commitment to community leadership. By providing screening support, navigation, training, and technical assistance, CBC works side by side with PHS and community partners to build capacity across the social care workforce. This collaboration supports organizations in delivering high‑quality social care support, deepening community engagement, and advancing wellness—rooted in real-world practice and shared learning. CBC is proud to stand with community providers every step of the way—because strong networks don’t just support systems, they support people.
