To improve access to infectious disease (HIV and hepatitis C) services for people in substance use facilities, Public Health Solutions, on behalf of the New York City Department of Health and Mental Hygiene, will fund up to four (4) high patient volume substance use treatment centers; one center in each of the following four NYC counties: Bronx, Brooklyn, Manhattan, and Queens, to provide HIV and viral hepatitis screening, diagnosis, linkage to care, and harm reduction services. The selected organizations will be expected to collect and report client level and aggregate data, review data periodically and identify areas for program improvement, and meet agreed-upon service delivery and performance targets.

Organizational Eligibility Requirements
Organizations applying for funding must meet all the following requirements:
- Be legally incorporated by the New York State Department of State as a not-for-profit corporation;
- Have federal tax-exempt status under Section 501 (c)(3) of the Internal Revenue Code;
- Operate a brick-and-mortar site in one of the following 4 counties: Bronx, Brooklyn, Manhattan, or Queens; and
- Be certified by New York State (NYS) Office of Addiction Services and Supports (OASAS) to provide crisis services, inpatient rehabilitation, outpatient, residential, and/or opioid treatment services.
