The PHS Healthy Families home visiting program supports NYC families from pregnancy through early childhood.
Overview
Healthy Families supports parents from pregnancy through their child’s 5th birthday. Our trained Family Resource & Support Specialists provide personalized guidance, parenting support, child development screenings, and referrals to local resources, all in the comfort of your home.
We help parents build strong, positive relationships with their children, have healthy pregnancies, and support their child’s growth and learning.
Families in Healthy Families participants see real results: healthier babies, stronger parenting skills, greater confidence, and children better prepared for school.

Who We Serve
You can enroll in Healthy Families anytime during pregnancy or before your baby turns 3 months old. We serve families in Brooklyn and Queens who reside in one of the following zip codes:

Impact
Healthy Families’ is shown to have important outcomes for both parents and children. This includes a 48% reduction in low-birth-weight deliveries among women who enroll before the 31st week of pregnancy, early literacy and success for children in school, and positive parenting practices and resilience.
